“The merger with HealthCare Connection makes Family Centers a more comprehensive organization equipped to provide a diverse range of services,” said Jan Dilenschneider, Family Centers’ board chairman. “And because HealthCare Connection and Family Centers also share similar missions, the two organizations will blend together seamlessly.” The merger is effective immediately, and HealthCare Connection’s four programs – the School Based Health Centers, Stamford CARES and Care to Care and the TLC Health Center at Kids in Crisis – will remain intact. “This partnership is very exciting, as HealthCare Connection’s programs are a wonderful complement to Family Centers’ services,” said the Rev. David R. Van Dyke, chairman of HealthCare Connection Board of Directors. “Because there is a natural overlap, we look forward to creating a more holistic approach to integrating routine health and mental health services.” Through its programs, HealthCare Connection meets the health and wellness needs of the Stamford community. Its School Based Health Centers are located in six middle and high schools in Stamford, and provide medical assessments, physicals, screenings, dental care, mental health counseling and other services to 1,900 students each year. Stamford CARES provides a host of services to help those living with HIV/AIDS, including case management, emergency financial assistance, referrals and support groups.
